Is Kratom Addictive? What You Need to Know About 7OH and MGM 16

A new class of substances is showing up at gas stations and smoke shops our team is already treating patients for it.

Dr. Jeff Harris, Medical Director for Recovery Centers of America at Greenville, recently spoke with WSPA 7News about two kratom-derived substances that are raising serious alarms: 7-hydroxymitragynine (7OH) and MGM 16.

Both substances are synthetic compounds derived from kratom, a plant product that has historically been sold as a dietary supplement. However, the concentrated, lab-processed versions that are now on shelves are more dangerous than people realize.

  • 7OH (7-hydroxymitragynine) produces psychoactive effects like fentanyl and carries a high potential for physical dependence and addiction
  • MGM 16 may be two to three times more potent than 7OH and researchers are still studying whether overdose-reversing medications like Naloxone are even effective against it
  • Both substances are largely unregulated, meaning there is no oversight of potency, purity, or dosage

Because 7OH and MGM 16 are sold in stores rather than on streets, many users don’t recognize them as high-risk substances. That accessibility is exactly what makes these substances so concerning. The barrier to first use is almost nonexistent, and dependence can develop quickly.

What Does Addiction to 7OH or MGM 16 Look Like?

Because these substances act on opioid receptors in the brain, addiction and withdrawal patterns can resemble opioid use disorder. Symptoms may include:

  • Strong cravings and compulsive use despite wanting to stop
  • Physical withdrawal when not using (nausea, muscle pain, anxiety, insomnia)
  • Increasing tolerance requiring higher doses for the same effect
